Position: Community Health Worker-Doula


Status: Full-Time


Reports To: Community Health Worker Lead

The Community Health Worker-Doula is responsible for providing ongoing outreach, health education, and family support services to at least 27-33 high-risk pregnant Women of Color in the specified geographic locations at a given time and effectively manages each participant’s individual needs and case file.


Roles and Responsibilities:


Outreach & Recruitment


 Provide community-based outreach and client recruitment to high-risk pregnant and

postpartum

 Women in need of perinatal and infant health care and supportive services.

 Process incoming referrals for program enrollment.

 Assist program participants in developing individualized care plans and a family

support plan.

 Promote comprehensive program services as appropriate through canvassing and

community health activities in the specified geographic locations of 33311 and 33313 zip codes.


Case Management


 Carry out case management activities including home visitation at least monthly or

more often based on participant level of need to include risk assessment, service referral and follow-up, resource distribution, health education, timely and accurate documentation, and case file maintenance.

 Educate participants on their prenatal care options including group prenatal care

 Health education, including the importance of waiting two years between

pregnancies.

 Screen pregnant and postpartum program participants for depression, intimate partner

violence, and substance use using selected evidence-based tools and provide

appropriate interventions based on identified needs.

 Administer the Ages and Stages Questionnaire with program participants and their

infants.

 Attend and complete all required training and continuing education programs.

 Work cooperatively and collaboratively with community partners and organizations

to meet the needs of the participant and family.


Knowledge, Skills, and Requirements:


 Must be knowledgeable about community, health, and social service resources.

 Must have strong customer service and interpersonal skills, demonstrating the ability to

work with participants and community partners and agencies in an effective manner with

sensitivity for diverse populations.

 Dependable transportation is required along with valid Florida driver’s license and

minimum auto insurance as required by state law. Local countywide travel is required.

 Use office equipment proficiently, i.e., personal computers including internet, email, and

databases.

 Proficient in the use of Microsoft Office applications, and Outlook.

 Working knowledge of the Healthy Start prenatal and infant risk screen forms and

processes.

 Excellent problem solving and communication skills, both verbal and written.

 Ability to relate to and communicate with a diverse community and population.

 Ability to be flexible and work independently.

 Ability to be flexible with regards to work location and work schedule, based on

participant needs.


Educational Requirements and Experience:


 Bachelor’s degree in Social Work or a related field preferred. A combination of education and with at least 3 years of experience directly related to the duties and responsibilities in public/community health will be considered.

 1-2 years of outreach, case management, or home health care experience.

 Requires evidence of specialized training and experience as a doula.


Other


 Must have a valid Florida driver’s license and reliable transportation.

 Must pass a Level 2 background and drug screening.
